<br />Contract Addendum – KCPHD Page 1 of 2 <br /> <br /> <br />CONTRACT ADDENDUM <br /> <br />KITTITAS COUNTY – KITTITAS COUNTY PUBLIC HEALTH DEPARTMENT <br /> <br /> THIS ADDENDUM , is made and entered into this __________ day of January, <br />2016, by and between KITTITAS COUNTY, a subdivision of the State of Washington, <br />herein after referred to as the “COUNTY” and the Kittitas County Public Health <br />Department, herein after referred to as “KCPHD.” <br /> <br />WHEREAS, the County and KCPHD entered into a certain contract entitled “Historic <br />Document and Preservation Funds Grant Agreement” in December of 2014; and <br /> <br />WHEREAS, said contract, at section 1.2, provided for an end date of December 31, 2015; <br />and <br /> <br />WHEREAS, the tasks contemplated have not yet been completed and the funds not yet <br />expended; and <br /> <br />WHEREAS, both the County and KCPHD wish to extend the agreement through March <br />31, 2016, so as to allow for the completion of the project. <br /> <br />NOW, THEREFORE, in consideration of the terms, conditions, covenants, and <br />performance contained herein, the parties hereto agree as follows: <br /> <br />Section 1.2 of the above-referenced contract is hereby amended to provide for an <br />end date of March 31, 2016 rather than December 31, 2015; and <br /> <br />All remaining sections of the above-referenced contract shall remain in full force <br />and effect. <br /> <br />IN WITNESS WHEREOF, the parties hereto have caused this Agreement to be <br />executed the day and year first herein above written. <br /> <br /> <br />COUNTY: KITTITAS COUNTY KITTITAS COUNTY PUBLIC HEALTH DEPT. <br /> 205 W. 5th, Room 108 507 N.
