KITTITAS COUNTY <br />DEPARTMENT OF PUBLIC WORKS <br />STAFF REPORT <br />AGENDA SESSION DATE: January 5, 2016 <br />ACTION REQUESTED: Officially enact Change Order #5 for the Tearaway <br />Paving Project. <br />BACKGROUND: In August of 2015 The Director of Public Works <br />obtained verbal authorization of Change order 95 in <br />the amount of $133,228.32. <br />The intent was to facilitate timely direction to the <br />contractor to proceed with the added and altered <br />work without a negative schedule impact. <br />Public Works Staff was directed to follow up with <br />the appropriate enabling documents, but through an <br />oversight, failed to do so. <br />This oversight was recently discovered while <br />processing final payment to the contractor for this <br />project. <br />INTERACTION: Public Works <br />RECOMMENDATION: Motion to enact Change Order #5 for the Teanaway <br />Paving Project. <br />HANDLING: Return a copy of the Resolution to Public Works. <br />Return two copies Change Order #5. <br />ATTACHMENTS: Change Order 45 <br />Resolution <br />LEAD STAFF: Roy Chance, Construction Manager <br />
