ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 14. (Effective dates January 1, 2015-
December 31, 2017)
BACKGROUND:
The Consolidated Contract Amendment 14
Amends Statements of Work for the following programs:
FPHS Communicable Disease and Support Capabilities-Effective July 1, 2017
Maternal and Child Health Block Grant- Effective January 1, 2015
Exhibit B-14 Allocations amends and replaces Exhibit B-13 Allocations:
Increase of $11,049 for a revised maximum consideration of $451,497
INTERACTION:
Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on November 20, 2017
Kittitas County Auditor’s Office: Reviewed and approved on November 21, 2017
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 14. (Effective dates January 1, 2015-
December 31, 2017)
