This Addendum takes effect on _______________ and modifies the Agreement between _____________________________________
and Shred-it USA Inc., parties to the Client Service Agreement for shredding services dated _________________ (the “Agreement”).
The parties hereby agree as follows:
Beginning on the effective date of this Addendum, the Term of the Agreement will be extended for an additional ____________ year(s) until
_____________________, and will continue thereafter for additional ____________ year period(s) unless terminated by either party, by
written notice, at least thirty (30) days prior to the end of the Term or any subsequent one year period. All other terms, conditions,
and obligations in the Agreement shall continue in full force and effect.
