ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Kittitas County Public
Health Administrator on the Consolidated Contract Amendment 12. (Effective dates: January 1,
2017- December 31, 2017)
BACKGROUND:
The Consolidated Contract Amendment 12 amends the Statement of work for the Office of
Immunization and Child Profile and no change in the total contract amount.

Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on July 14, 2017
Kittitas County Auditor’s Office: Reviewed and approved on July 20, 2017
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Kittitas County Public
Health Administrator on the Consolidated Contract Amendment 12. (Effective dates: January 1,
2017- December 31, 2017)
