Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />June 5, 2017 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners’ authorizes the signature of the Public Health <br />Administrator on the Consolidated Contract Amendment 11. (Effective dates: January 1, 2015- <br />December 31, 2017) <br /> <br />BACKGROUND: <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas County Public Health <br />Department. Adds a Statement of Work: <br /> Office of Immunization and Child Profile-Diamond Project-Effective January 1, 2017 <br />The Consolidated Contract Amendment 11 amends Statement of Work for the following <br />program: <br /> Office of Drinking Water Group A Program- Effective January 1, 2015 <br /> Office of Immunization and Child Profile-Effective January 1, 2017 <br />Exhibit B-11 Allocations, amends and replaces Exhibit B-10 Allocations as follows: <br /> Decrease of $300.00 for a revised maximum consideration of $358,397.00 <br /> <br />INTERACTION: <br />Kittitas County Board of Health: N/A <br />Kittitas County Prosecutor’s Office: Reviewed and approved on May 24, 2017 <br />Kittitas County Auditor’s Office: Reviewed and approved on May 31, 2017 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners’ authorizes the signature of the Public Health <br />Administrator on the Consolidated Contract Amendment 9.
