ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 11. (Effective dates: January 1, 2015-
December 31, 2017)
BACKGROUND:
The Consolidated Contract is the vehicle used by the Washington State Department of Health to
channel state authorized funding for programs and services to the Kittitas County Public Health
Department. Adds a Statement of Work:
Office of Immunization and Child Profile-Diamond Project-Effective January 1, 2017
The Consolidated Contract Amendment 11 amends Statement of Work for the following
program:
Office of Drinking Water Group A Program- Effective January 1, 2015
Office of Immunization and Child Profile-Effective January 1, 2017
Exhibit B-11 Allocations, amends and replaces Exhibit B-10 Allocations as follows:
Decrease of $300.00 for a revised maximum consideration of $358,397.00
INTERACTION:
Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on May 24, 2017
Kittitas County Auditor’s Office: Reviewed and approved on May 31, 2017
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment 9. (Effective dates: January 1, 2015-
December 31, 2017)
HANDLING:
Clerk of the Board send notice of approval to the Auditor’s Office.
