AMENDMENT TERMS AND CONDITIONS: <br />1. Contract expiration date of August 15, 2017 remains unchanged. <br />2. Contract performance period of July 1, 2015 through June 30, 2017 remains unchanged. <br />3. Change the overall contract amount from $468,984 to $514,490; an increase of $45,506 <br />4. Change the overall FY16/17 funding amount from $468,984 to $514,490; an increase of $45,506 <br />5. Replace the original Budget Sheet (Attachment D) with the revised Budget Sheet (Attachment D) attached. <br />This Amendment is incorporated in and made a part of the contract. Except as amended herein, all other terms <br />and conditions of the contract remain in full force and effect. Any reference in the original contract or an <br />Amendment to the “contract” shall mean “contract as amended”. The Department and Contractor acknowledge <br />and accept the terms of this Amendment as identified above, effective on the final date of execution below.
