ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Kittitas County Public
Health Department Administrator on the Data Use Agreement for Limited Data Set. (Effective:
upon signature for 5 years from effective date)
BACKGROUND:
The Limited Data set agreement is established to provide recipient with access to limited data set
for special department projects in accord with the HIPAA Regulations.

Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on April 14, 2017
Kittitas County Auditor’s Office: Reviewed and approved on April 17, 2017
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Kittitas County Public
Health Department Administrator on the Data Use Agreement for Limited Data Set. (Effective:
upon signature for 5 years from effective date)
