ACTION REQUESTED:
The Board of County Commissioners approves the signature of the Public Health Administrator on the
Consolidated Contract Amendment #10. (Effective dates: January 1, 2015-December 31, 2017)
BACKGROUND:
The Consolidated Contract is the vehicle used by the Washington State Department of Health to channel
state authorized funding for programs and services to the Kittitas County Public Health Department. The
Consolidated Contract Amendment 10 amends Statement of Work for the following program:
Office of Drinking Water Group B Program-Effective January 1, 2017
Office of Immunization and Child Profile-Effective January 1, 2017
Amends Statements of Work for the following programs:
Maternal and Child Health Block Grant-Effective January 1, 2015
Office of Drinking Water Group A Program-Effective January 1, 2015
Exhibit B-10 Allocations, Attached and incorporated by this reference, amends and replaces Exhibit B-9
Allocations as follows:
Increase of $25,514 for a revised maximum consideration of $358,697.00
Exhibit C-8 Schedule of Federal Awards, attached and incorporated by this reference, amends and replaces
Exhibit C-7

Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on October 20, 2016
Kittitas County Auditor’s Office: Reviewed and approved on October 21, 2016
RECOMMENDATION:
The Board of County Commissioners approves the signature of the Public Health Administrator on the
Consolidated Contract Amendment #10. (Effective dates: January 1, 2015-December 31, 2017)
