ACTION REQUESTED:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment #13. (Effective Dates: January 1, 2015-
December 31, 2017)
BACKGROUND:
The Amendment 13 Adds Statements of Work in Emergency Preparedness & Response-
(Effective July 1, 2017), FPHS Communicable Disease & Support Capabilities ( Effective July 1,
2017), and Supplemental Nutrition Assistance Program- Education-Effective October 1, 2017
Exhibit B-13 Allocations add and increases of $82,051 for a revised maximum consideration of
$440,448.00

Kittitas County Board of Health: N/A
Kittitas County Prosecutor’s Office: Reviewed and approved on September 25, 2017
Kittitas County Auditor’s Office: Reviewed and approved on September 27, 2017
RECOMMENDATION:
The Board of County Commissioners’ authorizes the signature of the Public Health
Administrator on the Consolidated Contract Amendment #13. (Effective Dates: January 1, 2015-
December 31, 2017)
