(Effective 01/01/2025-12/31/2027) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 6 amends the following: <br />Adds Statements of Work for: <br />Foundational Public Health Services: Effective July 1, 2025 <br />Office of Immunizations-ELC-Effective July 1, 2025 <br /> <br />Amends Statements of Work for: <br />Maternal and Child Health Block Grant: Effective January 1, 2025 <br />Office of People Services HR Public Health Infrastructure Grant- Effective January 1, 2025 <br />Office of Resiliency and Health Security-PHEP-Effective January 1, 2025 <br />Supplemental Nutrition Assistance Program-Education-Effective January 1, 2025 <br /> <br />Exhibit B-6 Allocations amends and replaces Exhibit B-5 Allocations as follows: <br />Increase of $199,433.00 for a revised maximum consideration of $4,608,529.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved 11.07.25. <br />Kittitas County Auditor’s Office: Reviewed and approved 11.17.25. <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approved the signature of the Public Health Director on <br />the Consolidated Contract Amendment 6.
