KITTITAS COUNTY <br />SHERIFF'S OFFICE <br />AGENDA STAFF REPORT <br />AGENDA DATE: July 6, 2021 <br />ACTION REQUESTED: Request to Acknowledge an Amendment to the Occupational <br />Medicine Services Agreement between KVH Workplace Health Medicine and Kittitas <br />County Sheriff's Office. <br />BACKGROUND: This Amendment, effective June 1, 2021, amends the Agreement <br />that was approved by the Board on April 7, 2021, between KVH Workplace Health <br />Medicine and Kittitas County Sheriffs Office. <br />The purpose of this Amendment is to ensure that the prices set in Exhibit "A" to the <br />original Agreement are corrected to reflect the correct pricing structure. <br />INTERACTION: Kittitas County Prosecuting Attorney's Office has reviewed and <br />approved this agreement. <br />RECOMMENDATION: Request to Acknowledge an Amendment to the Occupational <br />Medicine Services Agreement between KVH Workplace Health Medicine and Kittitas <br />County Sheriff's Office. <br />HANDLING: No handling <br />LEAD STAFF: Command Staff <br />
