Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />May June 3, 2019 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 8. (Effective date: 01/01/2018-12/31/2020) <br /> <br />BACKGROUND <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas County Public Health <br />Department. The amendment: <br />Adds Statements of Work for the following programs <br /> LSPAN-Local Strategies for Physical Activity and Nutrition- Effective February 19, <br />2019 <br />Amends Statements of Work for the following program: <br /> Childhood Lead Poisoning Prevention Program: Effective January 1, 2018 <br />Exhibit B-8 allocations amends and replaces Exhibit B-7 Allocations as follows: <br /> Increase of $58,500.00 for a revised maximum consideration of $335,654.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved on May 13, 2019 <br />Kittitas County Auditor’s Office: Reviewed and approved on May 16, 2019 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 8.
