Kittitas County <br />Public Health Office <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />3/1/2016 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners’ authorizes the signature of the Public Health Administrator on the <br />Evergreen Financial Services Agreement Amendment. (Effective date: Upon Signature- upon termination by <br />either party) <br /> <br />BACKGROUND: <br />The Evergreen Financial Services Agreement Amendment is entered into with Kittitas County Public Health <br />Department to allow Evergreen Financial Services to perform collection activity to recover unpaid accounts <br />for the Kittitas County Public Health Department. The amendment is needed to remove an end date on the <br />contract as this was an error on contractor part and is stated that can agreement may be terminated by either <br />party with 30 day written notice. <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: The Kittitas County Prosecutor’s Office reviewed and approved the <br />Collection Services Agreement Amendment on February 2, 2016. <br />Kittitas County Auditor’s Office: The Kittitas County Auditor’s Office reviewed and approve the Collection <br />Services Agreement Amendment on February 11, 2016. <br /> <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners’ authorizes the signature of the Public Health Administrator on the <br />Evergreen Financial Services Agreement Amendment. (Effective date: Upon Signature- upon termination by <br />either party) <br /> <br />HANDLING: <br />Clerk of the Board send notice of approval to the Kittitas County Auditor's Office. <br />Please note: Public Health will route originals to contractor, BOCC, Auditor's Office, and Public Health <br />files. <br /> <br /> <br />LEAD STAFF: <br />Candi Blackford <br />
