(Effective dates January 1, 2015- <br />December 31, 2017) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 14 <br />Amends Statements of Work for the following programs: <br /> FPHS Communicable Disease and Support Capabilities-Effective July 1, 2017 <br /> Maternal and Child Health Block Grant- Effective January 1, 2015 <br />Exhibit B-14 Allocations amends and replaces Exhibit B-13 Allocations: <br /> Increase of $11,049 for a revised maximum consideration of $451,497 <br /> <br />INTERACTION: <br />Kittitas County Board of Health: N/A <br />Kittitas County Prosecutor’s Office: Reviewed and approved on November 20, 2017 <br />Kittitas County Auditor’s Office: Reviewed and approved on November 21, 2017 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners’ authorizes the signature of the Public Health <br />Administrator on the Consolidated Contract Amendment 14.
