BOARD OF COUNTY COMMISSIONERS <br />COUNTY OF KITTITAS <br />STATE OF WASHINGTON <br /> <br />RESOLUTION <br />NO. 2017-_______ <br /> <br />RESOLUTION TO AUTHORIZE EXECUTION OF AN AMENDMENT TO THE CHILD <br />DEATH REVIEW CASE REPORTING SYSTEM DATA USE AGREEMENT BETWEEN THE <br />KITTITAS COUNTY PUBLIC HEALTH DEPARTMENT AND THE MICHIGAN PUBLIC <br />HEALTH INSTITUTE <br /> <br />WHEREAS, Kittitas County, through the Kittitas County Public Health Department (KCPHD), is <br />charged with the preservation, promotion, and improvement of health in the County; and <br />WHEREAS, KCPHD and the Michigan Public Health Institute (MPHI) entered into an Agreement <br />signed on April 4, 2016 to establish the terms and conditions for the collection, storage, <br />and use of data obtained from the case reviews of child deaths submitted by Child Death <br />Review teams in Kittitas County and entrusted to the MPHI as the Child Death Review <br />Case Reporting System; and <br />WHEREAS, the Parties desire to amend the Agreement by amending the introductory paragraphs of <br />Appendix B to explicitly list all 18 HIPPA identifiers.
