Kittitas County <br />Public Health Office <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />2/7/2017 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Administrator on the <br />Consolidated Contract Amendment #10. (Effective dates: January 1, 2015-December 31, 2017) <br /> <br />BACKGROUND: <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to channel <br />state authorized funding for programs and services to the Kittitas County Public Health Department. The <br />Consolidated Contract Amendment 10 amends Statement of Work for the following program: <br />Office of Drinking Water Group B Program-Effective January 1, 2017 <br />Office of Immunization and Child Profile-Effective January 1, 2017 <br />Amends Statements of Work for the following programs: <br />Maternal and Child Health Block Grant-Effective January 1, 2015 <br />Office of Drinking Water Group A Program-Effective January 1, 2015 <br />Exhibit B-10 Allocations, Attached and incorporated by this reference, amends and replaces Exhibit B-9 <br />Allocations as follows: <br />Increase of $25,514 for a revised maximum consideration of $358,697.00 <br />Exhibit C-8 Schedule of Federal Awards, attached and incorporated by this reference, amends and replaces <br />Exhibit C-7 <br /> <br />INTERACTION: <br />Kittitas County Board of Health: N/A <br />Kittitas County Prosecutor’s Office: Reviewed and approved on October 20, 2016 <br />Kittitas County Auditor’s Office: Reviewed and approved on October 21, 2016 <br /> <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Administrator on the <br />Consolidated Contract Amendment #10.
