KITTITAS COUNTY <br />AGREEMENT FOR SERVICES <br />This Agreement for Services (hereinafter "Agreement") is entered into by and between Kittitas County <br />(hereinafter "County"), a political subdivision of the State of Washington, and Apple Valley Counseling Services, <br />LLC d/b/a Valley Health and Counseling (hereinafter "Contractor"). <br />The purpose of this Agreement is as follows: To provide up to full-time, on -site mental health counseling <br />and substance use disorder professional services, as requested and scheduled by mutual written agreement, and <br />qualified practitioner services for behavioral -health assessment, medication management, care coordination, and <br />referral for individuals incarcerated at the Kittitas County Jail. <br />The term of this Agreement begins on the date of the last signature and continues for one (1) year, <br />unless sooner terminated under this Agreement.
