RESOLUTION TO CANCEL WARRANT
WHEREAS,the Board of Commissioners of Kittitas County Fire District No. 1 has been made aware
that the following warrant issued to volunteer Jacob Crandall,vendor number DOI54,needs to be
canceled:
Warrant##: O3514O 390,issued on 05/3O/2O25, in the amount of$20.82
WHEREAS,the warrant is not in the volunteer's possession and is considered lost, and
WHEREAS,the volunteer has completed the.Application for Duplicate Instrument Affidavit.
WHEREAS, it is verified that the warrant has not been cashed; d
WHEREAS, it is requested that the warrant be canceled and reissued..
THEREFORE,NOW VT y the Board of Commissioners of Kittitas County Fire
District No. 1,that the above listed warrant issued to Jacob Crandall,vendor number DOI54, is hereby
canceled, and is to be re-issued.
