(Effective 01/01/2025-12/31/2027) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 7 amends the following: <br /> <br />Exhibit A Statements of Work, includes the following statements of work, which are <br />incorporated by this reference and located on the DOH Finance SharePoint site in the Upload <br />Center at the following URL: <br /> <br />Global to Local Digital Health Initiative - Effective January 1, 2026 <br />Amends Statements of Work for the following programs: <br />Infectious Disease-Syndemic Prevention Services-SSP - Effective July 1, 2025 Maternal & Child <br />Health Block Grant – Effective January 1, 2025 <br />Office of Drinking Water Group A Program - Effective January 1, 2025 <br /> <br />Exhibit B-7 Allocations, attached and incorporated by this reference, amends and replaces <br />Exhibit B-6 Allocations as follows: <br />Increase of $121,206 for a revised maximum consideration of $4,729,735. <br /> <br />Exhibit B Allocations are attached only for informational purposes. <br /> <br />Exhibit C Federal Grant Awards Index, incorporated by this reference, and located in the <br />ConCon, Funding & BARS library at the URL provided above. <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved 01.07.26. <br />Kittitas County Auditor’s Office: Reviewed and approved 01.0726. <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approved the signature of the Public Health Director on <br />the Consolidated Contract Amendment 7.
