RESOLUTION TO CANCEL and REISSUE WARRANT(S)
the Board of Commissioners of Kittitas County Fire District No. I has been made aware
that the following four warrants issued to volunteer Douglas Buchanan, vendor number 0756 1,need to be
canceled:
Warrant #x 03514 0799, Issued on 9/3 12024,in the amount of$50.00
Warrant#: 0351401752,Issued on 1 d/31/2024, in the amount of 4..65
Warrant #; 0351402702, Issued on 11 22/21 4, in the amount of 50wtld
Warrant ; 0351403581, Issued on 12/2012024,in the amount of$50,00
WHEREAS, the warrants are considered last and have not been cashed; and
Douglas Buchanan has requested that the warrants be reissued and has completed the
required pp ication for Duplicate Instrument Affidavit;
THEREFORE,NOW BE IT RESOLVED by the Board of Commissioners of Kittitas County fire
District No. 1,that the above listed warrants ssu d to Douglas Buchanan, vendor number 07561 are
hereby cancel d, and that a new replacement warrant in than amount of$264.65 be issued.
