(Effective Dates: January 1, 2022, through December 31, <br />2024) <br /> <br />BACKGROUND: <br />The Consolidated Contract Amendment 14 amends the following: <br />Adds Statements of Work for the following programs: <br />Foundational Public Health Services (FPHS)- Effective July 1, 2023 <br />Office of immunization- Promotion of Immunization to Improve Vaccination Rates- Effective <br />July 1, 2023 <br /> <br />Amends Statements of Work for: <br />Office of Drinking Water Group A Program-Effective January 1, 2022 <br /> <br />Adds Exhibit B-14 Allocation: <br />Increase of $1,023,600.00 for a revised maximum consideration of $4,781,044.00. <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved July 6, 2023. <br />Kittitas County Auditor’s Office: Reviewed and approved on July 11, 2023. <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Director on the <br />Consolidated Contract Amendment 14.
