BOARD OF COUNTY COMMISSIONERS <br />COUNTY OF KITTITAS <br />STATE OF WASHINGTON <br /> <br />RESOLUTION <br />NO. 2022-_______ <br /> <br />RESOLUTION AUTHORIZING CONTRACT AMENDMENT 1 BETWEEN KITITAS COUNTY <br />PUBLIC HEALTH AND CENTRAL WASHINGTON DISABILITY RESOURCES <br /> <br />WHEREAS, Kittitas County Public Health Department (KCPHD) and Central Washington Disability <br />Resources (CWDR) entered into a professional services contract on November 5, 2020, <br />which is attached hereto and incorporated by this reference; and <br />WHEREAS, section 12 of the professional services contract allows for contract amendments that are <br />mutually agreed to by the parties and in writing; and <br />WHEREAS, KCPHD and the CWDR desire to amend the professional services contract by extending <br />the contract term to June 30, 2022, as detailed in the amendment that is also attached <br />hereto and incorporated by this reference; <br />NOW THEREFORE, BE IT RESOLVED that the Board of County Commissioners of Kittitas County, <br />Washington, hereby authorizes execution of contract amendment 1 between KCPHD and <br />CWDR extending the term of the professional services contract to June 30, 2022. <br /> <br />DATED this __________ day of ____________________, 2022, at Ellensburg, Washington. <br /> <br /> BOARD OF COUNTY COMMISSIONERS <br /> KITTITAS COUNTY, WASHINGTON <br /> <br /> __________________________________________ <br /> Chair <br /> <br /> __________________________________________ <br /> Vice-Chair <br /> <br /> __________________________________________ <br /> Commissioner <br />ATTEST: <br /> <br />_____________________________ <br />☐ Clerk of the Board <br />☐ Deputy Clerk of the Board
