Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />October 7th, 2021 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Director on the <br />Consolidated Contract Amendment 22 (effected dates 1/1/2018 – 12/31/2021). <br /> <br />BACKGROUND: <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas County Public Health <br />Department. The Amended Consolidated Contract amends the following: <br /> <br />Adds a statement of work for the following programs: <br />• Emergency Preparedness and Response PHEP- Effective July 1, 2021 <br />• Foundational Public Health Services-Effective July 1, 2021 <br /> <br />Amends Statements of Work for the following programs <br />• COVID 19 Coordinated Response- Effective July 1, 2020 <br />• LSPAN- Local Strategies for Physical Activity and Nutrition- Effective February 19, 2019 <br />• Maternal and Child Health Block Grant- Effective January 1, 2018 <br />• Office of Drinking Water Group A Program-Effective January 1, 2018 <br /> <br />Exhibit B-22 Allocations <br />• Increase of $182,929.00 for a revised maximum consideration of $3,779,101.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved 09/24/2021 <br />Kittitas County Auditor’s Office: Reviewed and approved 09/28/2021 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Director on the <br />Consolidated Contract Amendment 22 (effected dates 1/1/2018 – 12/31/2021). <br /> <br />HANDLING: <br /> <br />LEAD STAFF: <br />Tristen Lamb
