TING P AG <br />ITAS CO RK FO NTY JAIL <br />PRQGRAM <br />This Agreement, effective as of the date signed below, is made by and between Kittitas <br />County (hereinafter "the County") and Kittitas County Health Network ("Contractor"). The <br />County and Contractor agree as follows: <br />That the existing Agreement for the above referenced M.A.T. program, ("Agreement") <br />previously entered between the parties on July 6th, 2021, shall be modified as allowed <br />by section 12 (twelve) of that agreement. The purpose of this Modification is to allow for <br />change in required credentials of the Nurse Care Manager position due to unexpected <br />inability to recruit and hire a License Practical Nurse as had been anticipated by the <br />parties. The parties agree herein that the Nurse Care Manager position may filled by <br />any category of Nurse as defined in RGW 18.79 who is admitted to such practice in <br />accordance with that chapter. lt is further agreed that the financial provisions of the <br />existing agreement will be adjusted equitably pursuant to section 12.3, but that such <br />adjustment will involve only internal allocation and will not change the total amount of <br />compensation. <br />All terms, conditions, attachments, documents and exhibits, including the term of the <br />agreement as described in the original Agreement are incorporated herein by this <br />reference as fully as if set forth herein. <br />MODIFICATION OF THE EXISTING PERSONAL SERVICES AGREEMENT WITH KITTITAS COUNTY HEALTH <br />NETWORK FOR THE KITTITAS COUNry JAIL M.A.T.
