DESCRIPTION/JUSTIFICATION OF AMENDMENT: <br />This amendment adjusts the funds in the Budget Sheet (ATTACHMENT E) based on updated expenditures, also to include contract modification. <br />11. AMENDMENT TERMS AND CONDITIONS: <br />1. Contract expiration date August 15, 2021 remains unchanged as described on page 2 of this amendment. <br />2. Contract performance period of July 1, 2020 through June 30, 2021 remains unchanged as described on page 2 of <br />this amendment. <br />3. Change the overall FY21 funding amount from $316,236 to $342,736 an increase of $26,500 as described on page 2 <br />of this amendment. <br />4. Replace Original Budget Sheet Attachment E with attached Revised Budget Sheet Attachment E. <br />5. Change the FY21 CPD6 funding amount from $0 to $26,500; an increase of $26,500 as described on page 2 of this <br />amendment. <br />6. The FY21 Basic Service Operating funding amount remains unchanged of $280,426; as described on page 2 of this <br />amendment. <br />This Amendment is incorporated in and made a part of the contract. Except as amended herein, all other terms <br />and conditions of the contract remain in full force and effect. Any reference in the original contract or an <br />Amendment to the “contract” shall mean “contract as amended”. The Department and Contractor acknowledge <br />and accept the terms of this Amendment as identified above, effective on the final date of execution below.
