Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br /> <br />AGENDA DATE: <br />January 5, 2021 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 17 (effected dates 1/1/2018 – 12/31/2021). <br /> <br />BACKGROUND: <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas County Public Health <br />Department. The Amended Consolidated Contract amends the following: <br />Exhibit A Statements of Work are amended as follows: <br />Adds Statements of Work for the following programs <br />• Supplemental Nutrition Assistance Program-Education - Effective October 1, 2020 <br />• Division of Emergency Preparedness & Response PHEP - Effective July 1, 2020 <br />• LSPAN-Local Strategies for Physical Activity & Nutrition - Effective February 19, 2019 <br />• Maternal & Child Health Block Grant - Effective January 1, 2018 <br />• OICP CARES Enhanced Influenza Coverage Project - Effective July 1, 2020 <br />• OICP-Promotion of Immunizations to Improve Vaccination Rates - Effective July 1, <br />2020 <br /> <br />Exhibit B-17 Allocations amends and replaces Exhibit B-16 Allocations as follows: <br />• Increase of $155,495.00 for a revised maximum consideration of $2,309,403.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved December 15, 2020 <br />Kittitas County Auditor’s Office: Reviewed and approved December 17, 2020 <br /> <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 17 (effected dates 1/1/2018 – 12/31/2021). <br /> <br /> <br />HANDLING: <br /> <br /> <br />LEAD STAFF: <br />Tristen Lamb
