Yakima Neighborhood Health Services <br />12 South 8th St, PO Box 2605 <br />Yakima WA 98907-2605 <br />Phone (509) 454-4143 Fax (509) 454-3651 <br />www.ynhs.org <br /> Accredited by the Joint Commission Patient Centered Medical Home Level 3 <br /> <br /> <br />Health Benefit Exchange – Navigator Assistance <br />Memorandum of Understanding for Unfunded Partner <br />Yakima Neighborhood Health Services and Kittitas County Public Health Department <br /> <br />Purpose: <br />Yakima Neighborhood Health Services is a nonprofit organization which administers a contract <br />with the Washington Health Benefit Exchange (WAHBE) for the purpose of providing Navigator <br />services as described in the Affordable Care Act. Yakima Neighborhood Health Services desires <br />to establish a broad based network of cooperating organizations which similarly desire to enroll <br />as many persons as possible into Medicaid and Qualified Health Plans. Yakima Neighborhood <br />Health Services can provide certain enrollment related services of benefit to cooperating <br />organizations as a result of its experience and relationship with the Washington Health Benefit <br />Exchange. <br /> <br />Kittitas County Public Health Department also desires to enroll as many persons as possible <br />into insurance coverage. Kittitas County Public Health Department acknowledges and sees <br />benefit in participating in regional network of organizations whose purpose is also to maximize <br />enrollment under the ACA. Kittitas County Public Health Department desires to access, at no <br />cost, certain services which Yakima Neighborhood Health Services provides and are listed in the <br />agreement. <br /> <br />Yakima Neighborhood Health Services will provide: <br />• Assistance with troubleshooting applications (access to YNHS Enhanced User) <br />• Navigator network formation and coordination <br />• Management of communication between Navigator network and Washington Health <br />Benefit Exchange <br />• Monthly conference calls with Navigator network partners to address overall process and <br />implementation issues related to implementation of the Health Benefit Exchange and <br />Affordable Care Act <br />• Required process and systems training for Navigator network staff <br />• Inform Navigator network of required training in LMS <br />• Assistance with outreach, including co-coordination of outreach events <br />• Monitoring and reporting of enrollment activity for region and individual Navigator <br />organizations with Yakima Neighborhood Health Services Navigator network <br /> <br /> <br />Kittitas County Public Health Department will: <br />• Assure that Navigator network staff attends required training. <br />• Assure that all Navigator staff have 4 N 1 Attestation verifying that he/she meets <br />establish credentials and qualification, including:
