BOARD OF COUNTY COMMISSIONERS <br />COUNTY OF KITTITAS <br />STATE OF WASHINGTON <br /> <br />RESOLUTION <br />NO. 2019-_______ <br /> <br />RESOLUTION TO AUTHORIZE EXECUTION OF A LETTER OF COMMITMENT <br />WITH KITTITAS COUNTY HEALTH NETWORK <br /> <br />WHEREAS, Kittitas County, through the Kittitas County Public Health Department (KCPHD), <br />is charged with the preservation, promotion, and improvement of health in the <br />County; and <br />WHEREAS, Kittitas County Health Network (KCHN) a non-profit, intends to collaborate <br />across organizations to improve health outcomes for all members of the <br />community; and <br />WHEREAS, KCPHD and KCHN desire to collaborate, their efforts to further the shared goal of <br />improved community health and agree to commit those efforts as outlined in the <br />attached letter of commitment; and <br />NOW THEREFORE, BE IT RESOLVED that the Board of County Commissioners of Kittitas <br />County, Washington, hereby authorizes the Kittitas County Public Health <br />Department to execute a letter of commitment with Kittitas County Health <br />Network that is attached hereto, and incorporated herein by reference. <br />DATED this __________ day of ____________________, 2019, at Ellensburg, Washington. <br /> <br /> BOARD OF COUNTY COMMISSIONERS <br /> KITTITAS COUNTY, WASHINGTON <br /> <br /> __________________________________________ <br /> Chair <br /> <br /> __________________________________________ <br /> Vice-Chair <br /> <br /> __________________________________________ <br /> Commissioner <br />ATTEST: <br /> <br />_____________________________ <br />Clerk of the Board
