Kittitas County <br />Public Health Department <br /> <br />Agenda Staff Report <br /> <br />AGENDA DATE: <br />December 4, 2018 <br /> <br />ACTION REQUESTED: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 5. (Effective date: 01/01/2018-12/31/2020) <br /> <br />BACKGROUND <br />The Consolidated Contract is the vehicle used by the Washington State Department of Health to <br />channel state authorized funding for programs and services to the Kittitas County Public Health <br />Department. The amendment amends Statements of Work for the following program: <br /> Office of Emergency Preparedness and Response-Effective July 1, 2018 <br />Exhibit B-5 Allocations, attached and incorporated by this reference, amends and replaces <br />Exhibit B-4. Allocations as follow: <br /> Increase of $885.00 for a revised maximum consideration of $269,554.00 <br /> <br />INTERACTION: <br />Kittitas County Prosecutor’s Office: Reviewed and approved on November 14, 2018 <br />Kittitas County Auditor’s Office: Reviewed and approved on November 21, 2018 <br /> <br />RECOMMENDATION: <br />The Board of County Commissioners approves the signature of the Public Health Administrator <br />on the Consolidated Contract Amendment 5.
