KITTITAS COUNTY <br />Board of County Commissioners <br /> <br />AGENDA STAFF REPORT (AMENDED) <br /> <br />AGENDA DATE: November 20, 2018 <br /> <br />ACTION REQUESTED: Approve Claim of Interstate Batteries of Apple Valley. <br /> <br /> <br />BACKGROUND: The claimant, Interstate Batteries of Apple Valley (“Mr. Baker”), <br />claims damages of over $1,406.60 (as well as the cost of a rental van- <br />$537.50), for damage to the company’s van alleged caused by a <br />county employee driving a forklift. Kittitas County Claim # <br />201811010016. <br /> <br />INTERACTION: The incident alleged in the Claim involved a county employee using a <br />forklift to place a pallet inside of a van. The van was allegedly <br />damaged by the forklift during this process. The county employee <br />filled out an incident report consistent with the damages listed in the <br />Claim. <br /> <br />RECOMMENDATION: Approve Kittitas County Claim # 201811010016, in the total amount <br />of $1944.10 ($1,406.60 repairs + $537.50 rental van), contingent <br />upon receipt of a signed Release of all Claims from an authorized <br />representative of Interstate Batteries of Apple Valley. <br /> <br />HANDLING: Original – Clerk of the Board <br /> Copy – Auditor <br /> Copy – Prosecutor <br /> <br />ATTACHMENTS: Claim of Interstate Batteries of Apple Valley <br /> <br />LEAD STAFF: Christopher Horner, Deputy Prosecuting Attorney <br /> Patti Johnson, Public Works Director
