Grant funds are to be used exclusively for the following purpose: • Increase HPV vaccination rates by implementing strategies to identify opportunities to address Kittitas County's low completion rates for the 1:1:3 (Tdap, MCV, HPV) vaccine series. Conduct a post- implementation survey to measure the effectiveness of the implementation strategies and share learnings broadly. More generally, grant funds must be used only for purposes described in Section 501(c)(3) of the Internal Revenue Code of 1986, as amended (the "Code."). Grant funds may not be used to participate or intervene in any political campaign on behalf of or in opposition to any candidate for public office, including the publishing or distributing of statements. No part of this grant is earmarked to carry on propaganda or otherwise attempt to influence legislation. Grant funds shall be used for the purposes described in this agreement by 12/30/2019 (unless a written extension request is approved by the Foundation). You agree to return to the Foundation any grant funds unused as of this date (or an extended deadline, if applicable).
