DESCRIPTION/JUSTIFICATION OF AMENDMENT: <br />The Agreement Timeline is extended two months to allow for the pending approval of a controlled equipment <br />request waiver by DHS. Additions are denoted with grey highlight and deletions with strike through. <br />Key Personnel are also updated . <br />11. AMENDMENT TERMS AND CONDITIONS: <br />1. Change the Grant Agreement expiration date from June 30, 2018 to August 31, 2018. <br />2. The overall Grant Agreement amount of $50,548 remains unchanged. <br />3. Replace Amendment B Timeline Exhibit D with the Revised Timeline Exhibit D attached to this <br />amendment. <br />4. Update Article 1, Department Key Personnel Exhibit A as follows: <br />a. Change Tirzah Kincheloe to Dalton Gamboa, dalton.qamboa@mll.wa .gov , 253-512-7044. <br />b. Change Michael Alston's title to Program Coordinator. <br />C. Remove Deborah Henderson. <br />d. Add Sierra Wardell, Section Supervisor, s1erra.wardel l@m il.wa.gov, 253-512-7121. <br />This Amendment is incorporated in and made a part of the Grant Agreement. Except as amended herein, all other terms <br />and conditions of the Grant Agreement remain in full force and effect. Any reference in the _original Grant Agreement or an <br />Amendment to the "Grant Agreement" shall mean "Grant Agreement as amended".
