KITTITAS COUNTY <br />DEPARTMENT OF PUBLIC WORKS <br />AGENDA SESSION DATE: <br />ACTION REQUESTED : <br />BACKGROUND: <br />INTERACTION: <br />RECOMMENDATION : <br />HANDLING: <br />ATTACHMENTS: <br />LEAD STAFF : <br />STAFF REPORT <br />January 4,2016 <br />Officially enact Change Order #5 for the Teanaway <br />Paving Project. <br />In August of 20 15 The Director of Public Works <br />obtained verbal authorization of Change order #5 in <br />the amount of$133,228 .32. <br />The intent was to facilitate timely direction to the <br />contractor to proceed with the added and altered <br />work without a negative schedule impact. <br />Public Works Staff was directed to follow up with <br />the appropriate enabling documents, but through an <br />oversight, failed to do so. <br />This oversight was recently discovered while <br />processing final payment to the contractor for this <br />project. <br />Public Works <br />Motion to enact Change Order #5 for the Teanaway <br />Paving Proj ect. <br />Return a copy of the Resolution to Public Works. <br />Return two copies Change Order #5. <br />Change Order #5 <br />Resolution <br />Roy Chance, Construction Manager
