A RESOLUTION CANCELING A WARRANT -ro MARY WISE
WHEREAS Warrant#070123801115, dated 6/30/2011, in the amount o'l $692.5, 2,
made cut to Mary Wise was lost ard never cashed and
WHEREAS It is in the best interest of Kittitas County Water District#V7 to cancel
this warrant and reissue a new one
NOW THEREFORE be it resolved that the comm- issioners of Kittitas County
Water District#7 request that the Kittitas County Auditor cancel
Warrant #0701238015, dated 6/30/2011, in the amount of $692.62,
made out to Mary Wise, and to issue a new warrant for $692.62 to
Mary Wise.

issue to file in his office a written affidavit specifically alleging on oath that he is the proper owner,payee,
or legal representative of such owner or payee of the original instrument, giving the date of issue, the
number, amount, and for what: set-vices of claim or purpose the original instrument or series of instruments
of which it is a part was issued, and that the same has been lost or destroyed, and has not been paid.
