A RESOLUTION BY THE BOARD OF FIRE COMMISSIONERS
OF KITTITAS COUNTY FIRE PROTECTION DISTRICT NO 7, TO
CANCEL WARRANT NUMBER 411255243 DATED 10/31/2012 IN
THE AMOUNT OF TWO HUNDRED TEN DOLLARS AND .87 CENTS
($210.87) THE WARRANT WAS ISSUED PAYABLE TO FRED
MARION WARRANT IS LOST AND HAS NOT BEEN CASHED.
TO RE-ISSUE A NEW WARRANT TO FRED MARION
WHEREAS, the Board of Fire Commissioners of Kittitas County Dire District No
7, Kittitas County Washington, do hereby resolve to cancel warrant nu nber 411255243
in the amount of$210.87 dated 10/31/2012 paid to Fred Marion, the Warrant has been
lost. Resolve to reissue new warrant for Fred Marion.
WHEREAS, the above listed warrant was issued to Fred Marion and has been
lost. A new warrant needs to be reissued to Fred Marion..
NOW THEREFORE, be it resolved by the Board of Fire Commissioners of
Kittitas County Fire District# 7, to cancel Warrant#411.255243 in the amount of$210.87

Before a duplicate instrument is issued the issuing officer shall require the person making application for
its issue to file in his office a written affidavit specifically alleging on oath Haat he is the proper owner,
payee, or legal representative of such owner or payee of the original instrument, giving the date of issue,
the number, amount, and for what services of claim or purpose the original instrument or series of
instruments of which it is a part was issued, and that the same has been lost or destroyed, and has not been
paid.
