21. We are informed that Windy City Holdings, LLC is a Limited Liability Company (LLC). A copy of the duplicate original <br />of the filed LLC Certificate of Formation, the LLC Agreement, and all subsequent modifications or amendments must be <br />submitted to the company for review. <br />22.
