EXHO " A" <br />SCOPE OF WORK <br />The Contractor shall provide financial veterinary assistance of up to $300 per aid request for <br />residents of Kittitas County who self -attest to the following: <br />1) They are unable to pay for the needed veterinary care themselves, and <br />2) The animal requiring care is a companion animal that supports the applicant's mental <br />health <br />And self -attest to at least one of the following criteria: <br />1) They are a youth age Wand under, or <br />2) They area United States Veteran as defined by RCW 41.04.007, or <br />3) They area senior age 65 and over. <br />Additional project activities include: <br />Project Objective: Increasing Prevention and EnhancingAccess to Care and Services <br />Update Application for Assistance to include grant criteria <br />a Develop language/fliers advertising the new grant program <br />a Share the fliers with mental health clinics and vet offices in Kittitas County <br />o Accept applications for assistance & distribute funds to qualifying applicants <br />Kittitas County Professional Services Agreement <br />Page 15of19 <br />
