Exhibit B: School-to-Work Student Guide lnstructions <br />Thefottowing is a bitting guide for the Contractor as they work with students by fottowing the pages of the <br />Student Guide. As the fottowing sections are completed, the Contractor wit[ use Exhibit E: STW Bitting <br />Checktist for Student Guide, provided to the Contractor by DVR. <br />These instructions witt su pport gu idance to Schoot-to-Work (STW) contractors as they work with students <br />who are participating in STW. To begin the process of STWand to review the STW Student Guide, schedute a <br />STW lntake and Assessment meeting with the Student and the Student's Team. During th is meeting, the <br />STWStudent Guidewitt be shared with the student, goingthrough the process of STWand what the student <br />shoutd expect in their finat year of Transition. <br />Bitting lnstructions: <br />1. Bitt $2,OOO for the STW lntake Meeting once the fottowing pages are complete: <br />o Team Buitding and lnformation <br />o Team Contacts, Roles, and Responsibitities <br />2. Bitt $3,ooo for Assessment once the fottowing pages are complete: <br />Assessment pages may inctude the fottowing pages being completed, as appticabte for the student, as <br />each student mayget to their emptoyment goat in their own way. Att sources of information may be used to <br />determine the parts of the assessment. This may incl.ude job shadows, a Job Foundation Report through <br />DDCS, Work Based Learning through Pre-ETS, or any source to determine the needed information to <br />estabtish an emptoyment goat. The student and the student's team witl work together to determine this. <br />The main goat for this section is to get to an emptoyment goat and the neK steps for the student. <br />o STWAssessment <br />o Job Goal <br />o Next Steps and Team Actions <br />3. Bilt$1,000 forthe Job SeekerTools once the fottowing pages are complete: <br />o Job Seeker Toots (determined bythe student's team and have been compteted and shared with the <br />student and DVR) <br />4. Bilt $2,000 f or the Job Offer once the fottowing page is complete: <br />o Job Offer Detaits on the page titted "You were offered the job! Way to go!" <br />5. Bitt $1,000 for Employment Verification once the fottowing page is complete: <br />o Form is compteted on the page titted "Congratutations, you are emptoyedl" <br />6. Bitt $5,000 for Stabitization once the fottowing pages are comptete: <br />o Your Supports on the Job <br />o Job Stabitization Report <br />Ptease note there are additionat pages in the Student Guide that do not affect the above payments <br />and are intended to support the student.
