Exhibit B: Schoot-to-Work Student Guide Instructions The fottowing is a bitting guide for the Contractor as they work with students by fottowing the pages of the Student Guide. As the fottowing sections are compteted, the contractor witt use Exhibit E: STW Bil'ting Checktist for Student Guide, provided to the Contractor by DVR' These instructions witt support guidance to Schoot-to-Work (STW) contractors as they work with students who are participating in STW. To begin the process of STW and to review the STW Student Guide, schedule a STW lntake and Assessment meeting with the Student and the Student's Team. During th is meeting, the STWStudent Guidewitt be shared with the student, goingthrough the process of STWand what the student shoutd expect in their finat year of Transition. Bitting lnstructions: 1. Bitt $2,000 for the STW lntake Meeting once the fottowing pages are complete: o Team Buitding and lnformation o Team Contacts, Rotes, and Responsibitities 2. Bi[l $3,OOO for Assessment once the fottowing pages are comptete: Assessment pages may inctude the fottowing pages being completed, as appticabte for the student, as each student mayget to their emptoyment goat in their own way. Att sources of information may be used to determine the parts of the assessment. This may inctude job shadows, a Job Foundation Report through DDCS, Work Based Learning through Pre-ETS, or any source to determine the needed information to estabtish an employment goat. The student and the student's team witt work together to determine this. The main goat for this section is to get to an emptoyment goal, and the next steps for the student. r STWAssessment . Bilt $1,OOO f or the Job Seeker Tools once the fottowing pages are complete: o Job Seeker Toots (determined bythe student's team and have been completed and shared with the student and DVR) 4. BiLl $2,OOO for the Job Offer once the fottowing page is complete: o Job Offer Detaits on the page titted "You were offered the job! Way to go!" 5. Bitl $1,OOO for Emptoyment Verification once the fotl,owing page is complete: o Form is compteted on the page titted "Congratutations, you are emptoyed!" 6. Bilt $5,000 for stabitization once the fottowing pages are complete: o Your Supports on the Job r Job Stabitization Report please note there are additionat pages in the Student Guide that do not affect the above payments and are intended to support the student.
