Number of Students transferred into Adult Services or Job Placement with DVR' <br />(b) Exhibit D must be submitted a total of three (3) times to the DVR School-to-Work Transition <br />program lttrn"g"r or their designated representative throughout the course of the 2027 <br />transition conot following the bllow schedule and with the appropriate tab completed: <br />i. The ,'Aug 31 , 2026" tab shall be submitted on August 31 , 2026, or upon execution of the <br />Contract, whichever is later. <br />ii. The "March 31, 2027" tab shall be submitted on March 31 , 2027 . <br />iii. The ,,sept 30,2027" tab shall be submitted on september 30, 2027, or at the conclusion <br />of the Contractor's program, whichever is sooner' <br />(c) lf the above dates fall on a State-recognized holiday or weekend, Contractor may submit <br />Exhibit D on the nearest workday prior to the due date' <br />Monitoring. <br />a. The Department of social and Health Services, Division of Vocational Rehabilitation is required to <br />maintain a contract administration system which ensures that Contractors perform in accordance <br />w1h the terms, conditions, and specifications of their Contracts. DVR manages and monitors its <br />contracts through conducted risk assessments and monitoring plans. Authority-to conduct such <br />reviews is provideJ by Title 2 of the Code of Federal Regulations (CFR), 34 CFR 80.36' OFM <br />SAAM Policy 16.20.5-5, DSHS Administrative Policies No. 1 3' 1 1 and 19'50'30' <br />b. progress Meetings: DVR School-to-Work Transition Program Manager shallwork with Contractors <br />to pian meetings-which allow discussion of program progress and support methods to ensure <br />Students are receiving adequate services to ob.-tain employment. ln addition, Contractor may invite <br />DVR partners to attend locai County-initiated service provider meetings. DVR partners may include <br />tocat bVR Counselors, supervisors, and School-to-Work staff. <br />Subcontractors. <br />a.
