The intent of this phase is to ensure the Student and the Student's <br />Team collaborate to determine ii"norgh information is available to establish an employment goal' <br />establish the employment goal, ano nu'ilc an individualized roadmap of the student's transition from <br />their education to meaningfut community employment. This shall be accomplished lhrough the <br />following objectives: <br />(1) lntake <br />(a) At a minimum, the contractor shallwork with the student, the student's legal representative <br />o,. grrrJirn (unless they are their own), the cRP, and the DVR Counselor to: <br />i.Completeintakeformsasneeded'includingtheSDoP' <br />ii. Determine what Job Seeker Tools are needed for the Student's employment goal' <br />(2) Assessment <br />(a) Through the assessment process, the Contractor will collect information about the Student's <br />strengths, skills, and rrpjort. needed for success on the job. This .information will be based <br />on the Student,s schoot,-irome, and community work-re-lafed activities, interviews-with the <br />Student, family, school personnel, and others who are familiar with the Student' This <br />information may be outalneJ from Job Foundation report, work-based learning, school <br />ssessments, etc' <br />(b)Aiaminimum,thefollowingcriteriamustbeaddressed: <br />i. History of school attendance and reliability; <br />ii' Behavior support or potential strategies; <br />iii. Student Communication skills; <br />iv. Student learning Preferences; <br />v.
