A request for dispute resolution shall include: <br />(1) Name of the requester' <br />(2) Contractof's name, full address, phone number, and email' <br />(3) Contract number. <br />(4) Description of the issue in dispute. <br />(5) A statement describing the requester's position on the issue in dispute, including any <br />documentation that supports this position' <br />(6) Steps already taken to resolve the dispute' <br />e, The reviewer may request additional supporling documentation from either party to assist in <br />reaching a fair resolution. <br />f.
