Special Terms and Conditions <br />employment planning and connection; increase partnerships with school staff to complete student's <br />Job Foundation report with actionable next steps; and increase the number of students completing <br />transition programs with a job or secondary education connection. <br />(1) The Job Foundation report and process may be a source of information for the <br />Assessment in determining an employment goal for the Student. <br />f. "Job Placement" means locating, securing, and placing a Student into competitive integrated <br />employment that is mutually agreed upon by the DVR Counselor, the Contractor, and the Student <br />or the Student's Team. <br />m. "Job Stabilization" means the Student, the employer, the DVR Counselor and the Contractor <br />mutually agree that a Student placed in a Supported Employment position has demonstrated and <br />maintained satisfactory on-the-job performance and has the quantity and type of long-term <br />employment supports available to them that are needed to maintain satisfactory on-the-job <br />performance. <br />n. "Long Term Services" means ongoing support services provided by an entity other than DVR after <br />the Student obtains employment and exits DVR School -to -Work services. <br />o. "Monthly Progress Report" means progress reports that are to be filled out on a monthly basis and <br />shared with the Student's Team to demonstrate progress towards employment and summarize <br />monthly work being done with and on behalf of the Student. <br />p. "Assessment" means <br />(1) Identify a Student's unique work interests, abilities, and any competitive employment <br />barriers related to communication, mobility, work skills, work tolerance, self -direction <br />(cognition and learning), and interpersonal attitudes, skills, behavior, or self -care, etc.; <br />and <br />(2) Identify the nature and extent of support(s) and accommodations needed for the <br />Student to obtain and maintain competitive integrated employment. <br />q. "School -to -Work" means a program that supports a seamless transition for students with <br />intellectual/developmental disabilities from school to adulthood through employment services and <br />connecting students to the necessary resources for success. <br />r. "Service Delivery Outcome Plan (SDOP)" is a written plan jointly developed by the Student, DVR, <br />DVR Counselor, and CRP representative for obtaining School -to -Work services. The details of the <br />agreement are tailored to the unique needs of the Student and documented. <br />s. "Student" means an individual with an intellectual/developmental disability age 20 to 22 who is in <br />their high school transition program. <br />t. "Student Guide" means a guide designed for a Student and/or parent(s)/guardians to house School - <br />to -Work related documentation, information, assessments, activity logs, and notes. This guide is a <br />Student's 'road map' to ensure key activities and milestones are completed and organized. <br />u. "Student's Team" means any combination of people, including, but not limited to; educators, CRPs, <br />the Student's Legal Representative or Guardian, County Staff, DVR Counselor(s) who support a <br />Student as they move towards Job Placement and Job Stabilization. <br />DSHS Central Contract Services <br />6017CF County Program Agreement (10-31-2017) Page 3 <br />
