Welcome'. <br />School -to -Work Overview <br />The School -to -Work program is designed to assist you in finding a job before you exit school in June. In this <br />program, you will work directly with a job coach who will collaborate with you, your parent(s)/guardian(s), <br />school district staff, County Developmental Disabilities programs, the Division of Vocational Rehabilitation, and <br />Developmental Disabilities Administration. <br />This School -to -Mork Student Guide <br />This guide will be updated regularly and is designed to house your School -to -Work related information, <br />assessments, activity logs, notes, and much more. It will: <br />• Ensure key activities and milestones are completed and organized effectively. <br />• Support you in keeping important information in one place. <br />• Be a great resource to bring to collaboration meetings with your team. <br />This is a guide for students to ensure key activities and milestones have been completed and organized <br />effectively. This tool will support students in keeping important information in one place, and it will also be a <br />great resource to bring to collaboration meetings that occur with support partners and agencies. <br />School -to -Work Phases <br />You will be going through three phases for School -to -Work, all moving you toward your employment goal. <br />The phases are described throughout this guide. <br />Phase 1: intake and Assessment <br />This phase includes bringing your team together that will support and guide you through the program. Key <br />intake and assessment activities will take place to ensure a positive experience for you. <br />Phase 2: Job Placement <br />This phase includes conducting and capturing important job placement activities like employment goals, site <br />visits, job interviews, and more. <br />Phase 3: Employment <br />This phase serves as an exciting step to complete and capture all employment stabilization information like <br />employment verification information, completion of the first day of work, and much more! <br />DIVISION OF VOCATIONAL REHABILITATION I SCHOOL -TO -WORK I PAGE 3 <br />
