A request for dispute resolution shall include: <br />(1) Name of the requester. <br />(2) Contractor's name, full address, phone number, and email. <br />(3) Contract number. <br />(4) Description of the issue in dispute. <br />(5) A statement describing the requester's position on the issue in dispute, including any <br />documentation that supports this position. <br />(6) Steps already taken to resolve the dispute. <br />e. The reviewer may request additional supporting documentation from either party to assist in <br />reaching a fair resolution. <br />f. The Director shall issue a written decision to the Contractor within thirty (30) business days of <br />receipt of all information relevant to the issue. <br />g.
