A new Schedule A-5, Sfafem ent of Work, is added to prevent redundancy between the previous Schedule A(s), Sfafem ent(s) of Work, while also incorporating updates. ln the event of an inconsistency the revised and restated Schedule A-5, Sfafement of Work, shall supersede all previous Schedule A(s), Statement(s) of Work. The revised and restated Schedule A-5, Sfafement of Work, is attached hereto and by this reference is incorporated into the Contract. 5. A new Attachment 1-A, EXAMPLE - MOUD & MAUD in Jails Program Monthly Progress Report, is added to provide Contractor with the new reporting template to be required beginning July 1,2026. The updated Attachment 1-A, EXAMPLE - MOIJD & MAUD in Jails Program Monthly Progress Report, is attached hereto and by this reference is incorporated into the Contract' 6. This Amendment is effective July '1, 2026, ("Efiective Date"). 7. All capitalized terms not otherwise defined herein have the meaning ascribed to them in the Contract. 8. All other terms and conditions of the Contract remain unchanged and in full force and effect.
