ATTACHMENT "A" <br />SCOPE OF WORK <br />• Maintain sufficient FTE to provide established hours of Crisis Case Management (CCM) at Kittitas Valley <br />Healthcare. <br />■ Staff training to include use of Columbia -Suicide Severity Rating Scale, Involuntary Treatment Act law, and <br />the Daily Living Activities -20 functional assessment. <br />• Screen individuals who have been referred to CCM. <br />• Based on screening outcomes, refer individuals with behavioral health needs to appropriate care facilities, <br />treatment, or other applicable agencies. <br />■ Track and submit data as specified in Attachment "D" (Reporting) <br />Kittitas County Agreement for Services (rev. 5/14/25) <br />Page 3 of 22 <br />
