BOX 926 / 103 EAST 4TH AVE, SUITE 209 <br />ELLENSBURG, WA 98926 <br />509-925-9747 FAX: 509-962-3031 <br />brandon@belsmith.com <br />Per the description below, we present the following change order proposal #48 (COP#48). <br />During construction, it was discovered that the exhaust fans were omitted from the roofing <br />plan and therefore not incorporated into the PEMB (Pre -Engineered Metal Building) design. As <br />a result, the originally approved roof curbs were specified as flat, while the project requires <br />sloped roof curbs to properly integrate with the PEMB roof system. <br />This change order proposal includes: <br />Supply of new sloped roof curbs for the exhaust fans. <br />Material and labor to fabricate and install required roof framing supports for the new <br />curbs. <br />Additionally, a louver was omitted from the architectural elevations and only shown on the <br />HVAC drawings. Since it was not included in the PEMB design, additional work is required to <br />provide and install this louver. <br />This proposal includes all materials, labor, and equipment necessaryto install the louver and <br />properly integrate it into the existing wall system. <br />Total Cost added to contract: <br />Time extension required: 12 days <br />Sincerely, <br />Riley Eres, <br />Belsaas & Smith Construction <br />$9,755.41 + WSST <br />
