ATTACHMENT,,A" <br />SCOPE OF WORK <br />ln addition to providing all material and labor, the Contractor shall perform the following activities as indicated in <br />your Performance lndicators and Evaluation Workbook for the CWU Community Clinic project, attached to this <br />Scope of Work: <br />o Provide licensed and unlicensed staff support <br />o Offer mental health counseling services to youth and their families in Kittitas County <br />o Train and oversee its graduate students providing services <br />o Provide adequate facilities at CWU's Community Counseling Clinic <br />r Materials and supplies, including but not limited to surveys, books, and manuals <br />o Provide and maintain a HIPAA compliant healthcare portal <br />Kittitas County Agreement for Services (rev.5/La/251 <br />Page 3 of 32
